Write about your experience in your own words. Nurses, counselors, doctors, and other staff may be mentioned by name. Obvious email addresses, phone numbers, and links are stripped before storage; your own name or identifying information will likely be removed before publication, and a human reviews everything else for privacy.

Exactly what happens

You submit privately.
The form does not ask for your name, email, phone number, appointment date, or medical record number. The review database does not save your IP address. As with most hosted websites, the hosting provider may maintain routine security logs outside this review workflow.

A human reviews it.
Dr. Beeman checks every submission. Your name and other identifying information will likely be removed before publication; staff names may remain. Wording may be lightly edited for privacy, spelling, length, or clarity without changing its meaning. Approval is an editorial decision, not clinical verification.

Approved words become an image.
If approved, the redacted review is rendered as a PNG screenshot card labeled “Anonymous patient” and may be featured here. The original submitted text is then cleared from the database. Rejected submissions are deleted.

You stay in control.
You receive a one-time receipt code. Save it: entering that code below removes the private submission and any image currently hosted here. A public image may already have been copied or shared elsewhere.

Please leave the chart out of the chat.

Naming a nurse, counselor, physician, or other staff member is fine. Please do not include a patient or family member’s name, exact dates or locations, diagnoses, medications, treatment details, contact information, or other details that could identify a patient. If you include your own name or other identifying information, it will likely be removed before publication.

This box is not monitored for care, does not become part of a medical record, and does not establish a physician–patient relationship. The AI is not taking call. Contact the practice for clinical matters; call 911 for an emergency.
